Foundation is one of the largest Preferred Provider Organization (PPO) networks in Kern and Santa Barbara Counties. Our panel has over 1,500 credentialed physicians, ancillary providers, and hospitals. Foundation providers agree to accept discounted fees, physician peer review and utilization management as a means of maintaining quality care. Foundation emphasizes patient choice, and allows access to a broad selection of providers dedicated to delivering care in a cost effective manner. The discount structure is determined through negotiations with surgical facilities, hospitals, and ancillary providers by an independent actuary that is responsible to review all fees and ensure our competitive edge in the marketplace.
Foundation is affiliated with other foundations throughout California and is supported statewide with over 30,000 physicians, 400 hospitals and 7,000 ancillary providers.
Your employer has chosen the use of the Foundation’s benefit plan for your healthcare needs. Foundation plans are unique because they put you in charge of your medical expenses and offer discounted services; making it easier for you to make smart healthcare decisions. As a Foundation member you receive lower out of pocket costs when you choose to receive your healthcare from participating providers and/or hospitals. Please refer to our provider directory for a complete listing of contracted providers.

Language Assistance Program (LAP) SB 583
In 2003 the California Legislature enacted SB 853, which directed the Department of Managed Care (DMHC) and the Department of Insurance (DOI) to adopt regulations requiring healthcare service plans and health insurers to develop and implement a language assistance program (LAP) to provide services to their limited English proficient (LEP) enrollees.
As of April 1, 2009; limited English proficient (LEP) health plan members may access the Health Language Assistance Program by contacting their healthcare service plans listed on the back of the members ID card.
